Printing the following Forms from the Branch 35 Website
Click on the appropriate underlined line(s) above to select a form to print a PDF copy on your home printer.
You will need a pdf reader app. such as Adobe Reader or Preview (for Mac OS) on your computer for this PDF file to print.
For PCs when it prints, run your cursor to the bottom of the screen and a black window will appear. Click on the second icon from the left to activate your printer.
If you don’t have Adobe Reader on your computer, it is very easy and completely safe to download. You should have the latest version of Adobe Reader. On your Search provider, such as Google, enter “Adobe Reader.” It allows you to display and print PDF files. You might want to unclick the “Optional Offers.” Follow the instructions to download it to your computer. Then click the underlined line(s) below and follow the above instructions.
If you prefer to receive a Microsoft Word document of the form(s), please send an email to the Webmaster. You will be sent an email with the document as an attachment in the Microsoft Word format.
If you would prefer to receive a paper copy of either or both forms, send an email to the Webmaster, and it will be mailed to you at your residence address in the Branch 35 Membership Roster or at the address you indicate in your email.
